Title : Serial ultrasound findings after microwave ablation for benign breast lesions

Abstract:

Background: The ultrasound (US) imaging features of benign breast lesions (BBLs) after microwave ablation (MWA) are diverse and often mistaken by clinical doctors as malignant tumors.

Purpose: The purpose of this study is to describe the US imaging features at different periods after MWA for BBLs and explore possible factors related to complete absorption (CA).

Materials and methods: From August 2015 to October 2022, female BBLs patients who underwent percutaneous US-guided MWA were enrolled. Preoperative and postoperative follow-up US characteristics of all BBLs were collected. The US characteristics of ablated lesions were classified into five groups (CA, concentric circular echo (CCE), nodular uneven low echo (NULE), nodular even low echo (NELE), and sheet-like low echo (SLE)). A binary logistic regression was used to analyze the possible factors affecting CA after MWA.

Results: A total of 213 patients with 664 breast lesions were enrolled. The median follow-up time was 11 (4-22) months. At 6 months after ablation, CCE was the prominent US feature, accounting for 56%, followed by NULE (21.3%), CA (19.9%), SLE (1.9%), and NELE (0.9%). At 24 months after ablation, CA was the prominent US feature, accounting for 48.5%, followed by CCE (29.0%), NULE (14.3%), SLE (7.4%), and NELE (0.7%). Maximum diameter of the lesion ≤ 20 mm (odds ratio, 8.59; 95% CI: 2.32, 31.82; P =.001), almost entirely fat of breast composition (odds ratio, 2.61; 95% CI: 1.31, 5.20; P =.006), and number of ablated lesions ≥ 3 (odds ratio, 7.71; 95% CI: 2.08, 28.56; P =.002) were independent promoting factors related to CA.

Conclusion: The US features of BBLs after MWA are diverse. The main US feature of BBLs during the early stage after MWA is CCE, which evolves to CA over time.
